To have your patient received advanced imaging, please complete these 3 steps:

• Ask your client to call us at 920-560-3374 to set up an appointment with for the advanced imaging required.
• Complete the online Veterinary Referral Form
• Prior to your client’s appointment with us, please fax or email the patient’s pertinent medical records. Our general hospital inbox is veterinaryimagingfv@gmail.com
